Are 19 years or older

Patients are required to sign a Patient Consent Contract for treatment by volunteers.

Patients must have ID and provide documentation supporting their eligibility for services.

    • Digital radiographs

    • Extractions​

    • Full or partial dentures​

    • Hygiene visits

    • Fillings

    • Oral health education

    • Health maintenance

    • Prescription counseling

    • Prescription assistance

    Some medications can be dispensed on site through Proficient RX at no cost.

    No controlled or narcotic drugs will be prescribed.

Eligibility and Patient Forms Required

Eligibility

A current valid, government issued photo ID (driver's license, state ID, etc.)​

EITHER Proof of income for the most recent one-month period

  • Pay stubs, including year-to-date information, for all working household members OR Social Security Statement of “New Benefits”

  • Proof of all other household income (child support, worker’s compensation, unemployment, rental income, pension, etc.)

OR Your most recent tax filing

  • ​1040 or 1040ez (tax return)

  • Proof of non-filing status from the IRS for those who did not file taxes last year or have no income

  • The IRS can be reached at 1-800-829-1040

If you have neither proof of income or your most recent tax filing, we will need a Statement of Limited Income
